Permission Slip

I, _____________________________ give Smoky Valley Home Educators Co-op teachers permission to take my children, _______________________
_____________________________________________________________ on whatever field trips they deem educational, fun, or important to understanding the class material.  I understand that there will be two adults on each trip, I will be notified two weeks in advance of an upcoming field trip, that the leader will bring emergency information, and that all children riding in cars will have their own seat belt.

Conduct / Dress / Disciplinary Action Policy
